What to know before your first visit
A little preparation can help your appointment go more smoothly.
Your first appointment is the start of understanding what is causing your pain, injury, or limited movement, and what can be done to help. Southern Oregon Orthopedics provides specialty orthopedic care in Medford and Grants Pass, with a team focused on helping patients get clear answers and a plan for next steps.
Before your appointment
Prior to your first appointment, please make sure you have the following with you:
- Photo identification
- Current insurance information/card
- List of current medications
- List of allergies
- Medical history
- Referring physician’s notes, if applicable
- Previous MRI, CT, or X-ray imaging or nerve conduction studies, if applicable
- Copay
- Auto accident liability form, if applicable
- Workers’ compensation form, if applicable

What to expect at your visit
During your appointment, your provider will talk with you about your symptoms, medical history, activity level, and goals for treatment. Your visit may also include a physical exam and, if needed, imaging or additional testing to better understand your condition.
After your evaluation, your care team will review your diagnosis, explain your treatment options, and recommend next steps. Depending on your needs, your plan may include nonsurgical care, therapy, injections, bracing or splinting, additional imaging, or surgery.
Returning patients
Returning patients can use MyChart to schedule appointments, view health information, and manage parts of their care online.
